Eastern Case Study 1 - Primary care - calendar for older patients

Nuffield Road Medical Centre is a large GP practice in Cambridge covering a growing population on the rapidly-expanding north side of the city. Practice manager, Greta Evans, recognised the need for older patients in particular to have all their key medical and support information readily accessible in an easy-to-find place. She conceived of, gained funding for and produced a handy health information calendar which was sent out to every patient over the age of 75. The large print is easy to read. There is space for patients to record vital health information, emergency contacts, and next of kin. Key numbers such as the GP out-of-hours service are pre-printed, and there is a handy pocket to hold repeat prescription details. The design allows for confidential information to be hidden from view to the casual visitor. Additionally, each month contains useful health information ideas - ranging from "New Year - Cooking and Grandparenttning" to "Preparing for the Cold - Keep Well, Keep Warm".

The calendar was warmly received by the older patients, who not only found the calendar a useful aid to independence, but who also felt valued by the community in being sent a copy of the calendar.
