Dignity in Learning Disabilities Services in Derbyshire

Adults with learning disabilities across Derbyshire are being provided with 'Health Files'. These are personal records that they have about their own health. They are to help with communication, appointments and information. They give the person more control of their own health information and help health professionals to give information to the patient rather than the carer.
